新型コロナ感染症:アイコスのような「新型タバコ」にも重篤化リスクが:COVID-19:The severe symptoms risk of COVID-19 by “new nicotine delivery system” like IQOS too.

 流行が続く新型コロナウイルス(SARS-CoV-2)感染症(COVID-19、以下、新型コロナ感染症)だが、世界中で喫煙者は感染すると重症化するという警告が続出している。では、アイコスや電子タバコのような新型タバコに切り替えればリスクは下げられるのだろうか(この記事は2020/04/28の情報に基づいて書いています)。

喫煙で重症化するこれだけの証拠

 喫煙と新型コロナ感染症の重症化の関係について、WHOは「喫煙はCOVID-19にかかった際に重症化させるリスクがある」との事務局長談話を出し、ECDC(欧州疾病予防管理センター)は喫煙が新型コロナ感染症の感染と重篤化のリスクを高めると警告している。また、日本呼吸器学会は「新型コロナウイルス感染症とタバコについて」という声明の中で、喫煙は重症化の最大のリスクとしている。

 タバコを吸うと本当に新型コロナ感染症が重症化しやすいのだろうか。

 喫煙と新型コロナ感染症の関係は、中国で猛威をふるっていたころから指摘され、その後も同じような研究が続いて出されている。また「タバコ肺」と呼ばれるCOPD(慢性閉塞性肺疾患)が、新型コロナ感染症を重篤化させる独立した因子と指摘する論文なども多い(※1)。

 米国ハーバード大学などの研究グループが、新型コロナ感染症と喫煙の関係について5つの論文を選んで比較評価したシステマティックレビュー(※2)では、喫煙者は新型コロナ感染症で重篤化するリスクが1.4倍(RR=1.4、95%CI: 0.98-2.00)、ICUに入るリスクが約2.4倍、それぞれ高く、タバコを吸ったことのない人に比べて人工呼吸器を使うか死亡するリスクが2.4倍(RR=2.4、95%CI: 1.43-4.04)高くなるとした。

 また、イランのシーラーズ大学の研究グループが3月24日に発表した論文(※3)は、10論文を比較評価し、メタ解析で分析している。各論文で患者の喫煙歴のデータについてバラツキがあるとしつつも、喫煙によって新型コロナ感染症からの肺炎などの合併症にかかりやすくなることが示唆されるとした。

 喫煙と新型コロナ感染症の重症化の関係は、これまで蓄積されてきた知見から推測される。

 例えば、タバコ煙にさらされたマウスは、インフルエンザにかかりやすく、重篤化しやすかった(※4)。また、MERS(中東呼吸器症候群、MERS-CoV)でも喫煙者が重症化したという報告がある(※5)。

 インフルエンザの場合、システマティックレビュー20件、メタ解析12件を比較評価した論文(※6)によれば、喫煙者はタバコを吸わない人と比べ、インフルエンザにかかって入院するリスクは1.5倍(オッズ比)、重症化してICU(集中治療室)に入るリスクは2.2倍(オッズ比)だった。

 また、タバコを吸う人は、インフルエンザに一度かかって作られる抗体の作用やインフルエンザ予防ワクチンの効果がすぐに弱くなるようだ。抗体の効力が減退するスピードが速くなり、再びインフルエンザにかかる危険性が高くなってしまう(※7)。

なぜ加熱式タバコに重症化リスクがあるのか

 そもそもタバコから吸い込む有害物質は、気道や呼吸器の組織構造を変え、粘膜バリアや細胞組織などを破壊するし(※8)、肺などの臓器に備わっている免疫系の応答にも悪影響を及ぼす(※9)。

 タバコが免疫系に悪影響を及ぼすことはよく知られているが、タバコ製品に含まれるニコチンの作用という研究も多い。これは、ニコチンがドーパミンなどのカテコールアミン(Catecholamine)や副腎皮質ホルモン(コルチコステロイド、Corticosteroid)の分泌をうながし、免疫系の生体防御システムに影響を与えて感染に対する防御能力を抑制するからではないかと考えられている(※10)。

 つまり、タバコ製品に共通して含まれているニコチンが免疫系に作用し、新型コロナ感染症の重症化につながっているのかもしれないというわけだが、タバコ会社が「有害物質の低減」をアピールして製造販売しているアイコス(IQOS)、プルーム・テック(Ploom TECH)、グロー(glo)、パルズ(PULZE)といった加熱式タバコにもニコチンはしっかり含まれている

 加熱式タバコにも含まれているニコチンが、病気と関係するという研究はこの十数年間で数多く出てきた。

 米国カリフォルニア大学バークレー校などの研究グループによるゲノム解析研究では、肺がんとCOPDの感受性遺伝子とSNPs(一塩基多型、ゲノム上で一塩基だけが他のものに置き換わっている変異のうち、特定の集団の1%以上にみられる)が、ニコチン性アセチルコリン受容体に関連した領域の遺伝子群にあることがわかったという。また、ニコチンは受容体遺伝子に対し、メチル化という後天的な変化をさせるようだ(※11)。

 また、最新の研究(※12)によれば、ニコチンが肺の免疫細胞に悪影響を及ぼし、免疫細胞から産生されるインターロイキン(IL)22という炎症を抑えて細胞を治癒させるタンパク質の応答性を阻害することがわかったという。ニコチンは肺の免疫機能を損なうわけで、新型コロナ感染症に対する抵抗力を減退させる危険性がある。

 ところで、ニコチンと言えば神経系を保護し、むしろニコチンは脳にいいという研究があるが、そのほとんどはタバコ産業からの研究資金を得ている利益相反の疑いのあるものだ。現在では、喫煙がむしろアルツハイマー病(脳細胞の損傷や神経伝達物質の減少などが主な原因)や認知症を増加させることは周知の事実である(※13)。

 最近、フランスの脳神経学の権威が発表した論文(※14)で、ニコチンが新型コロナ感染症の感染リスクを下げるという内容のものが出たが、この研究者もニコチンの神経保護作用を研究することでタバコ産業から資金を得てきたことがわかっている。この論文中で述べられているニコチン性アセチルコリン受容体の抗炎症作用は以前から知られていたものだが、新型コロナ感染症の感染リスクとの関係は依然として未解明のままだ。

 ちなみに、SARS(重症急性呼吸器症候群)の時もタバコが感染患者を保護するという噂が出回ったことがある(※15)。だが、よく調べてみると、全くのデタラメだった。

 ニコチンが体内で代謝されるとコチニン、ノルニコチン、4-メチルアミノ-1-(3-ピリジル)-1-ブタノンに変わる。これらの物質が体内で反応すると、発癌性のあるニトロソアミンN’-ニトロソノルニコチン(NNN)、4-(メチルニトロソアミノ)-1-(3-ピリジル)-1-ブタノン(NNK)を発生させることがあるが、ニコチンとその代謝物である発がん性のあるニトロソアミン類は、脆弱な遺伝子に作用し、がんを発症させることがわかっている(※16)。

 ある物質は使用を誤れば毒にもなるし、その効果を適切に知って活用すれば薬にもなる。禁煙治療に使われるニコチンガムやニコチンパッチなどはその典型だろう。

電子タバコもやめておいたほうがいい

 ところで、加熱式タバコに含まれるニコチンの量は、紙巻きタバコよりごくわずかに少なく調整されている。これは喫煙者が加熱式タバコだけでは微妙に満足できない量になっていて、喫煙者を紙巻きタバコから加熱式タバコへ完全にシフトさせず、両方を吸わなければ我慢ならないようにしていると考えられる。

 スヌースという無煙タバコや噛みタバコにもニコチンが入っていて、スウェーデンの妊婦を調べた研究(※17)によれば、タバコ製品を何も使用しない人と比べ、スヌースの使用者の死産や早産のリスクが高いようだ。また、スヌースを使用すると、タバコをやめにくくなることが知られている(※18)。

 では、電子タバコはどうだろうか。

 これは同じ機構であるプルーム・テック(Ploom TECH)にも当てはまるが、グリセロールやプロピレングリコールをエアロゾル状に気化させて肺に吸い込む電子タバコは、肺や呼吸器に炎症を引き起こして肺炎になる危険性がある。2019年、米国で電子タバコによる肺炎が続発したのは記憶に新しい。

 米国で報告された電子タバコによる肺炎症状は、急性や慢性を含めて多岐にわたる。最新の論文(※19)によれば、電子タバコの肺への毒性を考えれば、少なくとも新型コロナ感染症が流行している間は喫煙をやめておいたほうがいいとしている。

 人にもよるが、タバコをやめると約8時間後に血液の中の酸素レベルは正常値に戻る。3日後には呼吸が楽になったと感じるはずだ。3ヶ月から9ヶ月後には肺の機能が戻り始める。また、ニコチンによる免疫系への影響は、禁煙すると数週間でなくなり、免疫系は正常に戻るという(※20)。

 科学的に信頼性の高いとされるコクラン(Cochrane)は、新型コロナ感染症のパンデミック時に効果的な禁煙方法を紹介している。日本語のサイトなので参考にしてほしい。
